Rehabilitative Services
We offer three major rehabilitation services which include physical therapy, occupational therapy and speech therapy. These three distinctive services work together to set up goals in order to achieve the best possible results for our Residents. In order to reach these goals, we:
- Start with a team of experienced professionals who are advocates committed to caring for our Residents;
- Identify short and long-term goals and create a care plan that maps out how to reach those goals;
- Provide daily treatments to foster growth and achievement;
- When appropriate, transition to a restorative nursing program to test durability of newly acquired skills and to continue to improve endurance.
- Involve family and at-home caregivers to keep them informed and to prepare them the support the Resident upon discharge, when possible.
A very unique service offered at the Clairemont Nursing and Rehabilitation, not found in many other facilities in the area, is the use of modalities in conjunction with traditional interventions, which include:
- Fall prevention
- Stroke rehabilitation
- Urinary incontinence
- Wound healing
- Peripheral neuropathy
- Rheumatoid arthritis
- Contracture management
- COPD (Chronic Obstructive Pulmonary Disease)
